Common use

Common side effects (≥1%) include nausea (11.0%), dizziness (5.8%), headache (5.6%), diarrhea (3.4%), insomnia (2.1%), and fatigue (1.9%). Less frequent adverse reactions may include vomiting, abdominal pain, flushing, increased blood pressure, tachycardia, anxiety, and tremors. Syncope has been reported in approximately 0.1% of patients. Visual disturbances and tinnitus occur rarely. Most side effects are mild to moderate in severity and tend to decrease with continued use. Drug interaction

Duratia is metabolized primarily by multiple CYP enzymes including CYP3A4, CYP2D6, and CYP2C19. Strong CYP3A4 inhibitors (ketoconazole, ritonavir, clarithromycin) increase dapoxetine exposure and concomitant use is contraindicated. Moderate CYP3A4 inhibitors (erythromycin, fluconazole, diltiazem) may increase dapoxetine levels—use with caution and consider 30mg dose. Concurrent use with other serotonergic drugs (SSRIs, SNRIs, triptans, tramadol) may increase risk of serotonin syndrome. Potent CYP2D6 inhibitors may increase dapoxetine exposure. Use with phosphodiesterase type 5 inhibitors may increase risk of orthostatic hypotension.

Missed dose

As Duratia is taken on an as-needed basis rather than regularly scheduled dosing, the concept of a “missed dose” does not apply in the conventional sense. If a dose is not taken within the recommended time frame before sexual activity, patients should not take the medication after sexual activity has begun. Do not take a double dose to make up for a missed timing. The next dose may be taken at the next appropriate opportunity, maintaining the minimum 24-hour interval between doses.

Overdose

In case of suspected overdose, seek immediate medical attention. Symptoms may include serotonin syndrome manifestations (agitation, confusion, diaphoresis, tachycardia, hyperthermia, muscle rigidity), syncope, dizziness, nausea, vomiting, and hypotension. There is no specific antidote for dapoxetine overdose. Treatment should consist of supportive measures including monitoring of vital signs and symptomatic treatment. Gastric lavage may be considered if presented early after ingestion. Activated charcoal may be administered if the patient presents within 1-2 hours of ingestion.
