Common use

Flixotide Nasal Spray is primarily indicated for the prophylactic treatment and management of seasonal and perennial allergic rhinitis in adults and children aged 4 years and older. It is particularly effective for patients who require continuous therapy throughout allergy seasons or year-round exposure to allergens. The spray is also used off-label for non-allergic rhinitis and as adjunctive therapy in patients with nasal polyps. Clinical studies demonstrate significant improvement in nasal symptom scores within 24-48 hours of initiation, with maximum benefit achieved after 3-7 days of regular use.

Dosage and direction

Adults and adolescents (12 years and older): The recommended starting dose is 2 sprays (100 mcg) in each nostril once daily. Maximum daily dose should not exceed 4 sprays (200 mcg) per nostril. For maintenance therapy, the lowest effective dose should be used.

Children (4-11 years): Initiate with 1 spray (50 mcg) in each nostril once daily. Maximum daily dose should not exceed 2 sprays (100 mcg) per nostril.

Administration technique: Shake gently before use. Prime the pump by spraying into the air until a fine mist appears (usually 6-8 actuations). Gently blow nose to clear nostrils. Tilt head slightly forward and insert nozzle into nostril, pointing toward the outer corner of the eye. Close the opposite nostril with finger and spray while breathing in gently through the nose. Repeat for the other nostril. Wipe nozzle clean after each use.

Precautions

Patients should be monitored for signs of nasal septum perforation, particularly with long-term use. Use with caution in patients with recent nasal surgery, nasal trauma, or nasal ulcers. Regular nasal examinations are recommended during prolonged therapy. May cause delayed wound healing. Patients with active or quiescent tuberculosis infections, untreated fungal, bacterial, or viral infections should use only under strict medical supervision. Not recommended for emergency treatment of acute bronchospasm. Growth velocity should be monitored in pediatric patients receiving long-term therapy.

Contraindications

Hypersensitivity to fluticasone propionate or any component of the formulation. Contraindicated in patients with untreated localized nasal infections. Should not be used during an acute asthma attack or other acute episodes where intensive measures are required. Not recommended for patients with severe hepatic impairment. Avoid use in patients with systemic fungal infections unless adequate antifungal therapy is instituted.

Possible side effects

Common (≥1/100): Nasal irritation, mild epistaxis, nasal dryness, sneezing attacks immediately after administration, headache, unpleasant taste, throat irritation.

Uncommon (≥1/1000): Nasal ulceration, pharyngitis, cough, nausea, nasal septum perforation (with long-term use).

Rare (<1/1000): Anaphylaxis, angioedema, bronchospasm, glaucoma, cataracts, adrenal suppression (with high doses), growth suppression in children, Cushing’s syndrome.

Very rare: Increased intraocular pressure, visual disturbances, nasal candidiasis.

Drug interaction

Concomitant use with strong CYP3A4 inhibitors (ketoconazole, ritonavir, clarithromycin) may increase fluticasone systemic exposure. Monitor patients for signs of hypercorticism. May potentiate effects of other corticosteroids. No significant interactions with nasal decongestants or antihistamines. Caution advised with other immunosuppressive agents due to increased infection risk.

Missed dose

If a dose is missed, administer as soon as remembered unless it is nearly time for the next dose. Do not double the dose to make up for the missed one. Resume regular dosing schedule. Consistent daily use is important for optimal efficacy, particularly during high pollen seasons.

Overdose

Acute overdose is unlikely due to low systemic bioavailability. Single doses up to 16 mg have been administered without adverse effects. Chronic overdose may lead to systemic corticosteroid effects including hypercorticism and adrenal suppression. Treatment should be symptomatic and supportive. Adrenal function tests may be indicated in cases of chronic overdose.

Storage

Store at room temperature (15-30°C). Do not freeze. Keep container tightly closed and protected from light. Discard 30 days after first opening, even if medication remains. Keep out of reach of children. Do not store in bathroom where moisture may affect medication stability. Do not puncture or incinerate container.

Reviews

Clinical studies: Multiple randomized controlled trials demonstrate 70-85% improvement in total nasal symptom scores compared to placebo. 89% of patients reported significant quality of life improvement in 12-week studies.

Patient feedback: Users report excellent control of morning nasal congestion and reduced reliance on oral antihistamines. Many appreciate the non-drowsy formula that doesn’t interfere with daily activities. Some users note initial nasal irritation that typically resolves within the first week of use.

Expert consensus: ENT specialists consistently rate fluticasone nasal spray as first-line therapy for moderate to severe allergic rhinitis due to its favorable efficacy and safety profile compared to systemic medications.
