Common use

It is not effective for other types of headaches, such as tension headaches. The goal of therapy is to abort the migraine attack, restoring the patient to a pain-free state within two hours.
Dosage and direction
Dosage is dependent on the formulation prescribed. Always use exactly as directed by your healthcare provider.
- Tablets: The recommended single dose is 25 mg, 50 mg, or 100 mg. If headache returns, a second dose may be taken after 2 hours, not to exceed 200 mg in 24 hours.
- Nasal Spray: A single spray (5 mg, 10 mg, or 20 mg) is administered in one nostril. If headache returns, a second dose may be taken after 2 hours, not to exceed 40 mg in 24 hours.
- Injection: A single 6 mg subcutaneous injection is administered. If headache returns, a second injection may be given after 1 hour, not to exceed two injections (12 mg) in 24 hours.
Take oral tablets with a full glass of water. Do not crush or chew the tablets. For the injection and nasal spray, carefully follow the specific instructions provided in the patient information leaflet for proper administration technique.
Precautions
Before using Imitrex, discuss your full medical history with your doctor. Important precautions include:
- Cardiovascular Risk: Imitrex can cause serious heart-related side effects, including heart attack or stroke. Your doctor will evaluate your cardiovascular status before prescribing it. Do not use if you have a history of heart disease, stroke, TIA, uncontrolled hypertension, or peripheral vascular disease.
- Chest/Throat/Jaw Pain: Sensations of tightness, pain, or pressure in the chest, throat, neck, or jaw are common side effects. While often non-cardiac, they must be evaluated by a physician to rule out serious heart disease.

Contraindications
Imitrex is contraindicated and must not be used in patients with:

Drug interaction
Inform your doctor of all medications you are taking, including prescription, over-the-counter, vitamins, and herbal products. Significant interactions include:
- Other Triptans and Ergotamines (e.g., dihydroergotamine, methysergide): Concomitant use is contraindicated due to a prolonged vasoconstrictive reaction.
- Monoamine Oxidase-A Inhibitors (MAOIs): Imitrex is metabolized by MAO-A; concomitant use or use within 2 weeks of stopping an MAOI is contraindicated.
- Selective Serotonin Reuptake Inhibitors (SSRIs) and Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs): Concomitant use may increase the risk of serotonin syndrome.
- Other vasoconstrictive agents: May have an additive effect.
Missed dose
Imitrex is not used on a scheduled basis; it is used only to treat an acute migraine attack as it occurs. Therefore, the concept of a “missed dose” does not apply. Use it at the onset of your migraine symptoms. If your headache returns after initial relief, you may take a second dose as directed by your doctor, respecting the minimum time interval and maximum daily dosage.
Overdose
In cases of overdose, the symptoms would be an exaggeration of the known side effects. Overdoses have been reported with ingestion of up to 400 mg. Symptoms could include hypertension, more pronounced cardiovascular events, seizures, and loss of consciousness. There is no specific antidote. In case of suspected overdose, contact a poison control center or emergency room immediately. Treatment should consist of supportive care, including continuous ECG monitoring for at least 20 hours.
Storage
- Store at room temperature between 20°C to 25°C (68°F to 77°F). Excursions permitted between 15°C to 30°C (59°F to 86°F).
- Protect from light and moisture.
- Do not store the injection autoinjector or nasal spray in a refrigerator or freezer.
- Keep all medicines out of the reach of children and pets.
- Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Dispose of unused medication through a medicine take-back program.
