Modafresh is a prescription-only wakefulness-promoting agent designed to treat excessive sleepiness associated with narcolepsy, shift work sleep disorder, and obstructive sleep apnea/hypopnea syndrome. Its active ingredient, modafinil, functions as a eugeroic—a class of compounds that promote alertness without the typical stimulant side effects. Developed to support sustained attention and reduce fatigue, Modafresh offers a targeted therapeutic approach for individuals requiring extended periods of wakefulness and mental clarity. It is not a substitute for healthy sleep hygiene but serves as an adjunct therapy under medical supervision.
Features
- Contains modafinil as the active pharmaceutical ingredient
- Available in 100 mg and 200 mg film-coated tablets
- Long duration of action, typically lasting 10–12 hours
- Low abuse potential compared to traditional stimulants
- Minimal impact on normal sleep architecture when used as directed
- Manufactured under cGMP (current Good Manufacturing Practices) standards
Benefits
- Promotes sustained wakefulness and reduces excessive daytime sleepiness
- Enhances cognitive functions including memory, attention, and executive function
- Improves quality of life for individuals with sleep disorders
- Supports shift workers in maintaining alertness during non-standard hours
- Low incidence of rebound hypersomnia or withdrawal symptoms
- Does not produce euphoria or significant cardiovascular stimulation
Common use
Modafresh is indicated for the improvement of wakefulness in adult patients with excessive sleepiness associated with narcolepsy, obstructive sleep apnea (OSA), and shift work disorder (SWD). In narcolepsy, it helps reduce cataplexy-associated episodes and sudden sleep attacks. For OSA patients, it is used adjunctively with primary treatments like CPAP therapy to address residual daytime sleepiness. Shift workers, such as healthcare professionals or emergency responders, may use Modafresh to maintain alertness during overnight or irregular shifts. It is not approved for use in pediatric populations or for general fatigue not linked to diagnosed sleep disorders.
Dosage and direction
The recommended dosage of Modafresh for narcolepsy or OSA is 200 mg taken orally once daily in the morning. For shift work disorder, 200 mg should be taken approximately one hour before the start of the work shift. Dosing may be adjusted based on individual response and tolerability, though doses above 200 mg daily have not shown increased efficacy. Tablets should be swallowed whole with water and may be taken with or without food, though a light meal is recommended to minimize potential gastrointestinal discomfort. Do not crush, chew, or split tablets.
Precautions
Patients should be cautioned about operating machinery or driving until they know how Modafresh affects them. Although the risk is lower than with traditional stimulants, there is a potential for psychoactive effects, dizziness, or visual disturbances. Use with caution in patients with a history of psychosis, depression, mania, or cardiovascular disease. Regular monitoring of blood pressure and heart rate is advised, particularly in those with pre-existing hypertension. Modafresh is not recommended for use during pregnancy unless clearly needed, and breastfeeding should be avoided during treatment.
Contraindications
Modafresh is contraindicated in patients with known hypersensitivity to modafinil or armodafinil, or any component of the formulation. It should not be used in individuals with symptomatic cardiovascular disease, including left ventricular hypertrophy, mitral valve prolapse, or recent myocardial infarction. Use is also contraindicated in patients with a history of drug abuse or addiction. Concomitant use with monoamine oxidase inhibitors (MAOIs) is contraindicated due to the risk of hypertensive crisis.
Possible side effects
Common side effects (≥1%) include headache, nausea, nervousness, anxiety, insomnia, dizziness, and diarrhea. Less frequently, dry mouth, indigestion, back pain, and rhinitis may occur. Serious but rare adverse effects include Stevens-Johnson syndrome, angioedema, multiorgan hypersensitivity reactions, and psychiatric symptoms such as aggression, hallucinations, or suicidal ideation. Any signs of rash, fever, or blistering should prompt immediate discontinuation and medical evaluation. Most side effects are mild to moderate and often resolve with continued use.
Drug interaction
Modafresh is a moderate inducer of CYP3A4/5 and may reduce the efficacy of drugs metabolized by this pathway, including oral contraceptives, cyclosporine, midazolam, and certain antiepileptics. It may also inhibit CYP2C19, potentially increasing levels of drugs such as diazepam, phenytoin, or omeprazole. Concurrent use with other CNS stimulants may amplify sympathetic effects. Hormonal contraceptives may be less effective; alternative or additional non-hormonal contraception is recommended during and for one month after Modafresh therapy. Always inform your healthcare provider of all medications, including over-the-counter drugs and supplements.
Missed dose
If a dose is missed, take it as soon as possible unless it is close to the next scheduled dose. Do not double the dose to make up for a missed one. For shift work disorder, if the dose is missed and the work shift has already begun, skipping the dose may be preferable to avoid potential sleep disruption later. Consistent daily timing is recommended for narcolepsy and OSA patients to maintain stable therapeutic effects.
Overdose
Symptoms of overdose may include insomnia, agitation, anxiety, confusion, nausea, tachycardia, hypertension, or hallucinations. In cases of suspected overdose, seek immediate medical attention. There is no specific antidote; treatment is supportive and may include symptomatic management, ECG monitoring, and activated charcoal if ingestion was recent. Hemodialysis is not expected to be effective due to high protein binding.
Storage
Store Modafresh tablets at room temperature (20–25°C or 68–77°F), in a tightly closed container, away from light, moisture, and heat. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging. Dispose of unused medication through a drug take-back program or according to local regulations.
