Common use

Dosage and direction
Dosage should be individualized based on indication and patient response. For most adults with erosive esophagitis, the recommended dose is 30 mg once daily for up to 8 weeks. For maintenance healing, 15 mg once daily is typical. H. pylori eradication regimens usually include Prevacid 30 mg twice daily alongside antibiotics such as amoxicillin and clarithromycin. Administer capsules whole; do not crush or chew. For patients with difficulty swallowing, capsules may be opened and sprinkled on a tablespoon of applesauce, followed by a glass of water. Alternatively, use the orally disintegrating tablet formulation.
Precautions
Use with caution in patients with severe liver impairment, as lansoprazole is metabolized hepatically. Long-term use (especially beyond one year) may be associated with hypomagnesemia; monitor magnesium levels periodically. Contraindications

Concomitant use with warfarin may require increased monitoring of INR due to potential interaction. PPIs can reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19. Avoid use with St. John’s wort or rifampin, which may decrease lansoprazole concentrations.
Missed dose
If a dose is missed, it should be taken as soon as remembered unless it is nearly time for the next dose. Do not double the dose to make up for a missed one. Consistency in daily dosing is important to maintain acid suppression.
Overdose
There is limited experience with lansoprazole overdose. Expected manifestations would be consistent with its pharmacologic action, such as hypochlorhydria. Supportive care and symptomatic treatment are recommended. Hemodialysis is not likely to be effective due to high protein binding.
