Dosage and direction
Dosage should be individualized based on the infection being treated, patient weight (where applicable), and clinical response. Administer with or without food.
- Bacterial Vaginosis: 2 g orally as a single dose.
- Trichomoniasis: 2 g orally as a single dose; sexual partners should be treated concurrently.
- Intestinal Amebiasis: Adults: 2 g once daily for 3 days. Children: 30 mg/kg (up to 2 g) once daily for 3 days.
- Amebic Liver Abscess: Adults: 1.5 g once daily for 5 days. Children: 30 mg/kg (up to 1.5 g) once daily for 5 days.
- Giardiasis: Adults: 2 g as a single dose. Children: 30 mg/kg (up to 2 g) as a single dose.
Always complete the full course of therapy, even if symptoms improve earlier.
Precautions
- Use with caution in patients with central or peripheral nervous system disorders, as nitroimidazoles may exacerbate neurological symptoms.
- Avoid alcohol consumption during treatment and for at least 72 hours after the last dose due to the risk of a disulfiram-like reaction (flushing, nausea, vomiting, tachycardia).
- May cause dizziness or drowsiness; patients should avoid driving or operating machinery until their response is known.
- Use during pregnancy only if clearly needed and potential benefits justify potential risks (Category C per FDA).
- Exercise caution in patients with hepatic impairment; dosage adjustment may be necessary.

Missed dose
If a dose is missed, take it as soon as remembered. If it is near the time of the next dose, skip the missed dose and resume the usual dosing schedule. Do not double the dose to catch up.
Overdose
Symptoms of overdose may include nausea, vomiting, ataxia, and neurological symptoms such as seizures. There is no specific antidote. Management is supportive and symptomatic, including gastric lavage if ingestion was recent and monitoring of neurological and hematological parameters.
