Common use

Tadalafil SX is indicated for the treatment of erectile dysfunction, the consistent or recurrent inability to achieve and/or maintain a penile erection sufficient for satisfactory sexual performance. It is also approved for the treatment of the signs and symptoms of benign prostatic hyperplasia (BPH). Its use is strictly for adult males. The medication requires sexual stimulation to be effective; it is not an aphrodisiac and does not increase libido.

Dosage and direction

For “as-needed” treatment of erectile dysfunction, the recommended starting dose is 10 mg, taken prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on individual efficacy and tolerability. Dosing should be limited to a maximum of one dose per 24-hour period. For daily use, the recommended dose is 2.5 mg or 5 mg taken approximately at the same time each day, without regard to timing of sexual activity. For the treatment of BPH, the recommended dose is 5 mg taken once daily at approximately the same time every day. The tablet should be swallowed whole with a glass of water; it may be taken with or without food.

Precautions

Prior to initiation of therapy, a thorough medical and cardiovascular assessment should be conducted to ensure the patient is healthy enough for sexual activity. Use with caution in patients with anatomical deformation of the penis (e.g., angulation, cavernosal fibrosis, or Peyronie’s disease), conditions that may predispose them to priapism (e.g., sickle cell anemia, multiple myeloma, leukemia), or bleeding disorders. Tadalafil SX may cause mild and transient decreases in blood pressure. Patients with left ventricular outflow obstruction (e.g., aortic stenosis, hypertrophic obstructive cardiomyopathy) or those with severely impaired autonomic control of blood pressure may be particularly sensitive to the vasodilatory effects. Use is not recommended in patients with uncontrolled hypertension or hypotension.

Contraindications

Tadalafil SX is contraindicated in patients with a known hypersensitivity to Tadalafil or any of the excipients in the formulation. Its concurrent use with any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or nitric oxide donors (e.g., amyl nitrite) is strictly contraindicated due to the risk of a profound and potentially fatal drop in blood pressure. Concomitant use with guanylate cyclase stimulators, such as riociguat, is also contraindicated as it may potentiate hypotensive effects.

Possible side effect

Tadalafil SX is generally well-tolerated. The most commonly reported adverse reactions are headache, dyspepsia, back pain, myalgia, nasal congestion, flushing, and limb pain. These are typically transient and mild to moderate in severity. Back pain and myalgia usually resolve within 48 hours. Less common side effects include dizziness, visual disturbances (non-arteritic anterior ischemic optic neuropathy - NAION has been reported rarely), hearing loss (sudden decrease or loss), palpitations, tachycardia, and prolonged erection lasting more than 4 hours (priapism). Priapism requires immediate medical attention to avoid permanent damage to penile tissues.

Drug interaction

  • Nitrates: Concomitant use is contraindicated. A washout period of at least 48 hours after taking Tadalafil SX is required before nitrate administration can be considered.
  • Alpha-blockers: Concomitant use can potentiate the blood-pressure-lowering effects. This combination should be initiated only if the patient is stable on their alpha-blocker therapy, and the Tadalafil SX dose should be limited to 5 mg for daily dosing or 20 mg for as-needed dosing, with a 48-hour separation from alpha-blocker dosing for the 20 mg dose.
  • Potent CYP3A4 Inhibitors: Drugs like ketoconazole, itraconazole, ritonavir, and clarithromycin can significantly increase Tadalafil plasma concentrations. For patients on a potent CYP3A4 inhibitor, the dose of Tadalafil SX should be limited to 10 mg no more than once every 72 hours for as-needed dosing, or 2.5 mg for daily dosing.
  • Alcohol: Excessive consumption (e.g., 5 units or more) in combination with Tadalafil may increase the potential for orthostatic signs and symptoms, such as increased heart rate, decreased standing blood pressure, dizziness, and headache.
  • Antihypertensives: Tadalafil SX may add to the blood-pressure-lowering effect of antihypertensive medications.

Missed dose

For the “as-needed” dosing regimen, a missed dose is not applicable as the medication is taken prior to planned sexual activity. For the once-daily dosing regimen, if a dose is missed, the patient should take it as soon as remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped. The patient should not take a double dose to make up for the missed one.

Overdose

In cases of suspected overdose, standard supportive measures should be employed. The primary expected hemodynamic effect is vasodilation and hypotension. Cardiovascular status and vital signs should be monitored actively. Management of hypotension should include placing the patient in the Trendelenburg position and administering intravenous fluids. If necessary, vasopressor agents may be considered. Hemodialysis is not expected to accelerate clearance as Tadalafil is highly bound to plasma proteins and not significantly eliminated in the urine.

Storage

Store Tadalafil SX tablets at room temperature (15°C to 30°C or 59°F to 86°F). Keep the medication in its original blister pack or container to protect it from light and moisture. Keep all medications out of the reach of children and pets. Do not use medicine beyond the expiration date printed on the packaging.
