It is used to manage symptoms such as wheezing, shortness of breath (dyspnea), and chronic coughing. It is a controller medication, not a rescue inhaler, and is part of a comprehensive COPD management plan that may include other bronchodilators, corticosteroids, pulmonary rehabilitation, and smoking cessation.

Dosage and direction

The recommended dosage for adults is two puffs (18 mcg of Tiotropium Bromide) once daily, at the same time each day. Do not exceed the prescribed dosage. Instructions for use:

  1. Remove the mouthpiece cover and check the mouthpiece for foreign objects.
  2. Shake the inhaler well immediately before each use.
  3. Breathe out fully, away from the inhaler.
  4. Place the mouthpiece between your lips and close your lips tightly around it.
  5. While breathing in deeply and slowly through your mouth, press down firmly on the canister to release the medication.
  6. Continue to breathe in slowly and hold your breath for up to 10 seconds if possible.
  7. Breathe out slowly.
  8. Wait about 30 seconds before taking the second puff, shaking the inhaler again.
  9. Replace the cap after use.
  10. Rinse your mouth with water after inhalation to help prevent potential side effects like dry mouth or oral candidiasis.

Precautions

  • Inform your physician of any history of kidney problems, glaucoma, prostatic hyperplasia, or bladder neck obstruction.
  • This medicine is not indicated for the initial treatment of acute episodes of bronchospasm (rescue therapy).
  • Immediate hypersensitivity reactions, including angioedema, may occur after administration. Discontinue immediately if such a reaction occurs.
  • As with any inhaled medication, paradoxical bronchospasm (a sudden worsening of breathing) may occur. If this happens, use a fast-acting rescue inhaler and stop using Tiova, contacting your doctor immediately.
  • Be aware of potential vision-blurring effects, which may be a sign of undiagnosed narrow-angle glaucoma.
  • Use with caution in patients with known cardiac rhythm disorders.

Contraindications

Tiova Inhaler is contraindicated in patients with a history of hypersensitivity to Tiotropium Bromide, atropine or its derivatives, including ipratropium, or any other component of the formulation.

Possible side effect

Common side effects may include:

  • Eye pain or discomfort, blurred vision, visual halos or colored images in association with red eyes from conjunctival congestion and corneal edema (symptoms of acute narrow-angle glaucoma)
  • Urinary retention, difficulty urinating
  • Signs of an allergic reaction: hives, rash, itching, swelling of the face, lips, tongue, or throat

Drug interaction

  • Concomitant use with other anticholinergic-containing drugs (e.g., ipratropium, aclidinium, umecidinium) may potentiate anticholinergic side effects and is not recommended.
  • While no specific pharmacokinetic drug interactions are known, always inform your doctor of all other medications you are taking, including prescription, over-the-counter, vitamins, and herbal products.

Missed dose

If you miss a dose, take it as soon as you remember on the same day. If it is nearly time for your next dose, skip the missed dose and take the next dose at your regular time. Do not take a double dose to make up for a missed one.

Overdose

An overdose may lead to an exacerbation of anticholinergic effects such as dry mouth, visual accommodation disturbances, tachycardia, and urinary retention. In case of suspected overdose, discontinue the medication and seek immediate medical attention or contact a poison control center. Supportive treatment should be administered.

Storage

  • Store at room temperature (15°C to 30°C).
  • Do not puncture or incinerate the canister, even when it is empty.
  • Keep away from direct sunlight, heat, and open flame.
  • Keep the canister away from eyes and skin.
  • Keep out of reach of children.
