Common use

Off-label uses may include attention-deficit/hyperactivity disorder (ADHD), bipolar depression (with caution and typically alongside mood stabilizers), and as an aid for smoking cessation (under the brand name Zyban). It is not typically considered a first-line treatment for anxiety disorders and may even exacerbate anxiety in some individuals.

Dosage and direction

Dosage must be individualized based on patient response and tolerance. The usual starting dose for Wellbutrin SR is 150 mg once daily, which may be increased to 150 mg twice daily after at least three days. For Wellbutrin XL, the initial dose is 150 mg once daily, which may be increased to 300 mg once daily after several days. The maximum recommended dose is 450 mg per day, divided into doses of no more than 150 mg each for IR formulations, or 400 mg for XL (as a single daily dose). Tablets should be swallowed whole and not crushed, chewed, or divided. Administration in the morning is recommended to minimize potential insomnia.

Precautions

Patients should be monitored for worsening depression, suicidality, or unusual changes in behavior, especially at the beginning of treatment or after dosage changes. Use with caution in patients with hepatic or renal impairment, dose adjustment may be necessary. May lower seizure threshold; use extreme caution in patients with history of seizure disorder, head trauma, or concomitant medications that lower seizure threshold. Contraindications

Wellbutrin is contraindicated in patients with a known hypersensitivity to bupropion or any component of the formulation. It must not be used in patients with a current or prior diagnosis of bulimia or anorexia nervosa due to increased risk of seizure. Contraindicated in patients undergoing abrupt discontinuation of alcohol or sedatives. Concomitant use with monoamine oxidase inhibitors (MAOIs) is contraindicated; at least 14 days should elapse between discontinuation of an MAOI and initiation of Wellbutrin.

Possible side effects

Common side effects (≥1%) include dry mouth, nausea, insomnia, headache, dizziness, and constipation. Less frequently, tachycardia, rash, sweating, and tremor may occur. Significant but less common adverse reactions include seizures (dose-dependent), hypertension, and psychiatric reactions such as anxiety, agitation, or hallucinations. Allergic reactions, including angioedema and anaphylaxis, have been reported. Any new or worsening symptoms should be reported to a healthcare provider promptly.

Drug interaction

Wellbutrin is metabolized primarily by CYP2B6. Concomitant use with CYP2B6 inhibitors (e.g., ticlopidine, clopidogrel) may increase bupropion exposure. Inducers of CYP2B6 (e.g., ritonavir, carbamazepine) may decrease levels. May interact with drugs that lower seizure threshold (e.g., antipsychotics, antidepressants, tramadol). Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is close to the time of the next scheduled dose. In that case, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling the dose to make up for a missed one is not recommended, as it may increase the risk of adverse effects, including seizure.

Overdose

Overdose may lead to seizures, hallucinations, tachycardia, and loss of consciousness. Fatal outcomes have been reported, especially with mixed overdoses involving multiple drugs. Symptoms may be delayed in onset. Management is supportive and symptomatic; there is no specific antidote. Airway protection and ECG monitoring are essential. Activated charcoal may be considered if presented early. Contact a poison control center immediately for guidance.

Storage

Store at room temperature (20–25°C or 68–77°F), with excursions permitted between 15–30°C (59–86°F). Keep in a tightly closed container, away from light, moisture, and excessive heat.
