Dosage and direction

Adults:

Renal impairment:

  • CrCl >30 mL/min: no adjustment needed
  • CrCl 10–30 mL/min: 250–500 mg every 12 hours
  • CrCl <10 mL/min: 250–500 mg every 24 hours

Administer at the start of a meal to enhance absorption and reduce gastrointestinal upset. Complete the full course of therapy even if symptoms improve earlier to prevent resistance.

Precautions

  • Use with caution in patients with hepatic dysfunction; monitor liver enzymes periodically during prolonged therapy
  • May cause antibiotic-associated colitis; evaluate for diarrhea during and after treatment
  • Avoid in patients with mononucleosis due to increased risk of rash
  • Use with caution in patients with renal impairment; dose adjustment required
  • Prolonged use may result in fungal or bacterial superinfection
  • Not recommended during late pregnancy or while breastfeeding due to potential for neonatal diarrhea or candidiasis

Contraindications

  • History of hypersensitivity to amoxicillin, clavulanate, or any other beta-lactam antibiotic
  • Previous history of Augmentin-associated cholestatic jaundice or hepatic dysfunction
  • Patients with phenylketonuria (certain chewable tablets and suspensions contain phenylalanine)
  • Concomitant use with disulfiram (due to alcohol content in some suspensions)

Missed dose

Overdose

Symptoms may include gastrointestinal disturbances (nausea, vomiting, diarrhea), electrolyte imbalances, and crystaluria. Management is primarily supportive with maintenance of hydration and electrolyte balance. Hemodialysis may enhance elimination of both components.
