Dosage and direction

The recommended initial dose for Feldene is 20 mg administered as a single daily dose. For many patients, this dose may be sufficient for maintenance therapy. The dosage may be divided if desired; however, daily doses larger than 20 mg are not recommended. The smallest effective dose should be used for the shortest duration consistent with individual patient treatment goals. The capsule should be taken with a full glass of water and should not be crushed, chewed, or broken. To minimize the potential for esophageal irritation or gastrointestinal upset, it is advised to take Feldene with food or milk.

Precautions

Patients should be informed of the signs and symptoms of gastrointestinal ulceration and bleeding and be instructed to report any such occurrences. Feldene should be used with caution in patients with a history of gastrointestinal disease, pre-existing asthma, hypertension, cardiac dysfunction, or renal impairment. Periodic monitoring of hemoglobin, renal function, and liver enzymes is advisable during prolonged therapy. Patients should avoid alcohol consumption during treatment, as it may increase the risk of gastrointestinal bleeding. Caution is advised when engaging in activities requiring alertness, such as driving, as dizziness or drowsiness may occur.

Contraindications

Feldene is contraindicated in patients with known hypersensitivity to piroxicam, aspirin, or other NSAIDs. It should not be used in patients who have experienced asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs. Severe, sometimes fatal, anaphylactic reactions to NSAIDs have been reported in such patients. It is also contraindicated in the setting of coronary artery bypass graft (CABG) surgery and in patients with active peptic ulcer disease or a history of recurrent ulcer/bleeding.

Possible side effect

Common side effects may include nausea, epigastric pain, dyspepsia, diarrhea, constipation, flatulence, dizziness, drowsiness, and headache. Serious side effects requiring immediate medical attention can occur and include signs of gastrointestinal bleeding (e.g., black, tarry stools, vomiting blood), signs of liver problems (e.g., nausea, fatigue, jaundice), signs of heart problems (e.g., shortness of breath, swelling of feet or ankles), signs of kidney problems (e.g., change in amount of urine), severe skin reactions, and signs of an allergic reaction (e.g., rash, itching/swelling, severe dizziness, trouble breathing).

Drug interaction

Feldene may interact with several other medications. Concomitant use with other NSAIDs or aspirin may increase the risk of gastrointestinal toxicity. It may reduce the antihypertensive effect of ACE inhibitors, angiotensin II receptor blockers, and beta-blockers. It can increase the serum level and risk of toxicity from lithium and methotrexate. Concurrent use with anticoagulants like warfarin increases the risk of bleeding. It may enhance the nephrotoxic effect of cyclosporine and decrease the natriuretic effect of furosemide and thiazides. Patients should provide their doctor with a full list of all medications, including over-the-counter drugs and supplements.

Missed dose

If a dose of Feldene is missed, it should be taken as soon as it is remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped and the regular dosing schedule resumed. Do not take a double dose to make up for a missed one.

Overdose

Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, gastrointestinal bleeding, coma, or acute renal failure. Respiratory depression and seizures may occur with significant overdose. Management is supportive and symptomatic. There is no specific antidote. Gastric lavage or activated charcoal may be considered if presented soon after ingestion. Forced diuresis, alkalization of the urine, hemodialysis, or hemoperfusion are unlikely to be beneficial due to the drug’s high protein binding.

Storage

Store Feldene capsules at room temperature, between 20°C to 25°C (68°F to 77°F), in a tightly closed container. Keep the medication away from light, excess heat, and moisture (do not store in the bathroom). Keep all medications out of the reach of children and pets.
