Precautions
- Cardiovascular Status: Patients with pre-existing cardiovascular disease were excluded from pre-marketing clinical trials. Sexual activity carries a potential cardiac risk. Therefore, Levitra Professional, like all PDE5 inhibitors, is not recommended for use in men for whom sexual activity is inadvisable due to their underlying cardiovascular status. A thorough cardiovascular assessment is recommended prior to initiating any treatment for ED.
- Prolonged Erection (Priapism): If an erection persists for more than 4 hours, the patient should seek immediate medical attention. Priapism, if not treated promptly, can lead to permanent damage to the penile tissues and long-term erectile dysfunction.
- Sudden Hearing Loss: Cases of sudden decrease or loss of hearing have been reported post-marketing in temporal association with the use of PDE5 inhibitors, including vardenafil. It is not possible to determine if these events are related directly to the drug or other factors. Patients experiencing such symptoms should discontinue use and seek prompt medical attention.
- Non-Arteritic Anterior Ischemic Optic Neuropathy (NAION): A rare but serious condition involving sudden vision loss in one eye has been reported rarely post-marketing. Risk factors include a “crowded” optic disc, heart disease, diabetes, and hypertension. Patients who have experienced NAION in one eye have an increased risk of NAION in the other eye.
- Alpha-blockers & Blood Pressure: Concomitant use with alpha-blockers can lead to symptomatic hypotension in some patients (see Drug Interactions).
- Hepatic and Renal Impairment: Use with caution and dose adjustment is recommended in patients with hepatic impairment. No dose adjustment is required for patients with mild to moderate renal impairment; caution is advised in patients with severe renal impairment (CrCl < 30 mL/min).
Contraindications
Levitra Professional is absolutely contraindicated in the following patient populations:
- Patients taking any form of organic nitrates (e.g., nitroglycerin, isosorbide mononitrate or dinitrate) or nitric oxide donors (e.g., amyl nitrite “poppers”) in any form, either regularly or intermittently. Concomitant use can cause a profound and life-threatening drop in blood pressure.
- Patients taking guanylate cyclase stimulators, such as riociguat.
- Patients with a known hypersensitivity to vardenafil or any of the excipients in the formulation.
- Patients with severe hepatic impairment (Child-Pugh C).
- Patients with end-stage renal disease requiring dialysis.
- Patients with a history of myocardial infarction, stroke, life-threatening arrhythmia, or hypotension within the last 6 months.
- Patients with unstable angina or angina occurring during sexual intercourse.
- Patients with hereditary degenerative retinal disorders, including retinitis pigmentosa.
Possible side effect
Like all potent medications, Levitra Professional can cause side effects, although not everybody gets them. Most are mild to moderate and transient.
Very Common (≥1/10) and Common (≥1/100 to <1/10):
- Headache
- Flushing (reddening of the face and neck)
- Nasal congestion (stuffy nose)
- Dyspepsia (indigestion)
- Dizziness
Uncommon (≥1/1,000 to <1/100):
- Palpitations
- Tachycardia (increased heart rate)
- Hypotension (low blood pressure)
- Syncope (fainting)
- Visual disturbances (e.g., blurred vision, changes in color vision)

Rare (≥1/10,000 to <1/1,000):

Drug interaction
The concomitant use of Levitra Professional with other medications can lead to significant interactions.
- Nitrates: Absolute Contraindication. Concomitant use can cause a severe, potentially fatal, hypotension.
- Alpha-blockers (e.g., doxazosin, tamsulosin): Not Recommended. Co-administration can lead to symptomatic postural hypotension, dizziness, and syncope. If co-administration is necessary, vardenafil should be taken at least 6 hours after the alpha-blocker, and treatment should be initiated at the lowest dose (5 mg standard vardenafil, not Professional).
- Potent CYP3A4 Inhibitors:
- Ketoconazole, Itraconazole: Maximum dose should not exceed 5 mg of standard vardenafil in a 24-hour period. Levitra Professional (20 mg) is not recommended.
- Ritonavir, Indinavir: Maximum recommended dose is 2.5 mg of standard vardenafil in a 72-hour period.
- Erythromycin, Clarithromycin: Maximum dose should not exceed 5 mg of standard vardenafil in a 24-hour period.
- Other Antihypertensives: Vardenafil has additive effects when co-administered with other antihypertensive medications. Blood pressure should be monitored.
- Guanylate Cyclase Stimulators (e.g., Riociguat): Absolute Contraindication. May cause hypotension.
Missed dose
Levitra Professional is not intended for chronic daily dosing but is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Simply take one tablet when you plan to engage in sexual activity, ensuring at least 24 hours have passed since your last dose. Do not take a double dose to make up for a missed opportunity.
Overdose
In cases of overdose exceeding the maximum recommended dose, the expected effects would be an extension of the drug’s known adverse effects profile, primarily severe hypotension, syncope, and prolonged erection. Standard supportive measures should be employed, including placing the patient in the supine position with the legs elevated to manage hypotension. Hemodialysis is not expected to accelerate clearance as vardenafil is highly protein-bound. Management of priapism should be immediate and may require corporal irrigation; this is a urological emergency.
