Common use

Terramycin Ophthalmic Ointment is primarily indicated for the topical treatment of superficial ocular infections caused by susceptible organisms, including bacterial conjunctivitis and blepharitis. In dermatological practice, it is used for the treatment of susceptible superficial skin infections, such as impetigo, folliculitis, and for the prophylaxis of infection in minor cuts, burns, and abrasions. It is also widely utilized in veterinary medicine for similar conditions in animals, particularly for eye infections in cats and dogs.

Dosage and direction

For Ophthalmic Use: A small amount (approximately a ½-inch ribbon) of ointment should be applied into the conjunctival sac of the affected eye(s) every 3 to 4 hours, or more frequently as directed by a physician. The frequency of dosing may be reduced as the infection is brought under control. For Topical Skin Use: Apply a thin layer to the affected area 2 to 4 times daily. The area may be covered with a sterile bandage if deemed appropriate. General Directions: Wash hands before and after application. To prevent contaminating the tube tip, avoid direct contact with the eye, skin, or any other surface. For the eye, gently pull down the lower eyelid to create a pouch, apply the ointment, close the eye for 1-2 minutes, and roll the eyeball to ensure distribution. Blurred vision may occur immediately after application.

Precautions

  • This is a prescription medication. Use only under the supervision of a healthcare professional.
  • Prolonged use may result in overgrowth of non-susceptible organisms, including fungi. If new infections appear during therapy, discontinue use and consult a physician.
  • Patients should be advised not to wear contact lenses while exhibiting signs and symptoms of an ocular infection nor during the course of therapy with this drug.
  • For external use only. Not for injection or otic (ear) use.
  • If the condition worsens, or if irritation, redness, swelling, or pain persists or increases, discontinue use and notify a physician promptly.

Contraindications

Terramycin is contraindicated in patients with a known history of hypersensitivity to oxytetracycline, polymyxin B, or any other component of the formulation. It is also contraindicated in those individuals who have shown hypersensitivity to other tetracycline antibiotics, as cross-sensitivity may occur. Its use in the treatment of mycobacterial infections, viral diseases, or fungal infections is not indicated.

Possible side effect

While generally well-tolerated, local reactions are the most commonly reported side effects.

  • Ophthalmic Use: Transient burning or stinging upon application, blurred vision, eye discomfort, itching, mild erythema (redness) of the eyelid or conjunctiva.
  • Dermatological Use: Localized irritation, pruritus (itching), erythema, and rash at the site of application.
  • Rare: Allergic reactions, including skin rash, urticaria (hives), angioedema, and photosensitivity (more associated with systemic tetracycline use but a potential risk with topical application if significant systemic absorption occurs).

Drug interaction

Formal topical interaction studies have not been conducted. However, theoretical interactions exist.

  • The bactericidal action of polymyxin B may be antagonized by concomitant use of other topical drugs that inhibit bacterial cell wall synthesis.
  • Concurrent use of other topical products on the same site may cause physical or chemical interactions, potentially leading to irritation or inactivation of either product. A separation of application times by several minutes is advised.
  • While systemic absorption is minimal, caution is advised in patients taking oral anticoagulants (e.g., warfarin), as tetracyclines have been reported to depress plasma prothrombin activity.

Missed dose

If a dose is missed, it should be applied as soon as it is remembered. However, if it is almost time for the next scheduled dose, skip the missed dose and resume the usual dosing schedule. Do not apply a double dose to make up for a missed one.

Overdose

Acute overdose from topical application is unlikely due to minimal systemic absorption. Excessive application may increase the frequency and severity of local adverse effects, such as irritation. In the event of accidental ingestion, seek medical attention or contact a Poison Control Center immediately. Symptoms of oral tetracycline overdose can include nausea, vomiting, and diarrhea.

Storage

Store at controlled room temperature, 20°-25°C (68°-77°F). Do not freeze. Keep the tube tightly closed when not in use. Protect from light and excessive heat. Keep out of reach of children and pets. Do not use after the expiration date printed on the tube and carton.
