Voveran, containing the active ingredient Diclofenac, is a highly effective nonsteroidal anti-inflammatory drug (NSAID) prescribed for the management of pain and inflammation. It represents a cornerstone in therapeutic regimens for a variety of musculoskeletal and painful inflammatory conditions. By inhibiting cyclooxygenase (COX) enzymes, it effectively reduces the production of prostaglandins, the key mediators of pain, fever, and inflammation. This makes it a first-line option for clinicians seeking to restore patient mobility and improve quality of life.

Features

  • Active Ingredient: Diclofenac Sodium or Diclofenac Potassium.
  • Drug Class: Nonsteroidal Anti-Inflammatory Drug (NSAID); COX inhibitor.
  • Available Formulations: Oral tablets (including sustained-release), dispersible tablets, topical gels, suppositories, and injectable solutions.
  • Mechanism of Action: Potent, non-selective inhibition of both cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes.
  • Bioavailability: High oral bioavailability; potassium salt offers faster absorption than the sodium salt.
  • Half-life: Approximately 1-2 hours for the parent compound, with active metabolites extending the therapeutic effect.

Benefits

  • Provides rapid and potent relief from moderate to severe pain, facilitating a return to daily activities.
  • Effectively reduces swelling, stiffness, and joint inflammation associated with arthritic conditions.
  • Offers multiple administration routes (oral, topical, rectal) for tailored treatment and improved patient compliance.
  • Helps manage acute painful episodes, such as post-surgical pain or injuries, reducing the need for opioid analgesics.
  • Improves overall functional capacity and range of motion in patients with chronic inflammatory diseases.
  • The availability of a topical formulation provides targeted relief with minimized systemic exposure.

Common use

Voveran is indicated for the treatment of pain and inflammation in a wide range of conditions. Its primary uses include the symptomatic treatment of osteoarthritis and rheumatoid arthritis, where it alleviates joint pain and stiffness. It is also highly effective in managing ankylosing spondylitis. For acute musculoskeletal conditions, it is prescribed for sprains, strains, and tendinitis (e.g., frozen shoulder, tennis elbow). It serves as an analgesic for acute gout flares and provides relief from post-operative pain and inflammation following dental or orthopedic procedures. The topical gel is specifically used for the localized relief of pain in osteoarthritis of superficial joints, like the knees and hands.

Dosage and direction

Dosage is strictly individualized based on the indication, formulation, and patient response. The following are general guidelines; medical supervision is mandatory.

  • Oral Tablets (Diclofenac Sodium): For rheumatoid arthritis and osteoarthritis, the typical dose is 100-150 mg per day, administered in 2 or 3 divided doses.
  • Dispersible Tablets (Diclofenac Potassium): Often used for acute pain (e.g., dysmenorrhea). The typical dose is 50 mg three times daily. This salt is absorbed more quickly.
  • Topical Gel (1% or similar): Apply 4g of gel to the affected area 4 times daily. The total dosage should not exceed 32g per day for all affected joints.
  • Suppositories: Commonly used at a 100mg dose, often at bedtime for round-the-clock relief.
  • General Direction: Oral formulations should be taken with a full glass of water and with food, milk, or an antacid to minimize the risk of gastrointestinal upset. Do not crush, chew, or break sustained-release tablets. For topical use, apply to intact skin only; do not use with occlusive dressings. Wash hands before and after application.

Precautions

  • GI Risk: NSAIDs, including Voveran, increase the risk of serious gastrointestinal adverse events such as bleeding, ulceration, and perforation, which can be fatal. This risk is higher in the elderly, those with a history of peptic ulcer disease, and those on concomitant anticoagulant therapy.
  • CV Risk: Long-term use may increase the risk of serious cardiovascular thrombotic events, including myocardial infarction and stroke. This risk may increase with duration of use and is higher in patients with pre-existing cardiovascular disease.
  • Renal Effects: Long-term administration can result in renal papillary necrosis and other renal injury. Use with extreme caution in patients with impaired renal function, heart failure, liver dysfunction, those taking diuretics or ACE inhibitors, and the elderly.
  • Hepatic Effects: Borderline elevations of liver function tests may occur in up to 15% of patients. Rare cases of severe hepatic reactions have been reported. Monitor liver function periodically during long-term therapy.
  • Pregnancy: Avoid use during the third trimester of pregnancy as it may cause premature closure of the fetal ductus arteriosus. Use during the first and second trimesters should only be if the potential benefit justifies the potential risk to the fetus.
  • Asthma: Use with caution in patients with asthma, as it may exacerbate the condition or induce bronchospasm.

Contraindications

Voveran is contraindicated in the following scenarios:

  • Known hypersensitivity (e.g., anaphylactic reactions, asthma, urticaria, or other allergic-type reactions) to diclofenac or any components of the drug product.
  • Patients who have experienced asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
  • In the setting of coronary artery bypass graft (CABG) surgery.
  • Significant renal impairment or active peptic ulcer disease.
  • Third trimester of pregnancy.

Possible side effect

Like all medications, Voveran can cause side effects, although not everybody gets them. Common side effects include:

  • Gastrointestinal: Dyspepsia, nausea, vomiting, abdominal pain, diarrhea, flatulence, constipation.
  • Central Nervous System: Headache, dizziness, vertigo.
  • Dermatological: Rash, pruritus.
  • Other: Elevated liver enzymes, fluid retention, edema.

Serious side effects requiring immediate medical attention:

  • Signs of GI bleeding (e.g., black, tarry stools; vomiting blood or coffee-ground material).
  • Signs of liver problems (e.g., nausea, fatigue, lethargy, jaundice, right upper quadrant tenderness).
  • Signs of heart problems (e.g., shortness of breath, chest pain, swelling of legs/feet/ankles).
  • Signs of an allergic reaction (e.g., skin rash, swelling of the face/tongue/throat, severe dizziness, trouble breathing).
  • Signs of skin reactions (e.g., blisters, fever, itching).

Drug interaction

Voveran has a significant potential for drug interactions. Inform your doctor of all medications you are taking.

  • Anticoagulants (e.g., Warfarin): Increased risk of serious bleeding.
  • Other NSAIDs/Aspirin: Concomitant use increases the risk of GI toxicity with no therapeutic benefit.
  • ACE Inhibitors/ARBs/Diuretics: Voveran may reduce the antihypertensive and natriuretic effect of these drugs, potentially leading to renal impairment.
  • Lithium: NSAIDs can decrease lithium clearance, leading to increased plasma lithium levels and potential toxicity.
  • Methotrexate: NSAIDs can reduce the tubular secretion of methotrexate, increasing its blood levels and risk of toxicity.
  • Cyclosporine: Increased risk of nephrotoxicity.
  • SSRIs/SNRIs (e.g., Citalopram, Duloxetine): Concomitant use may increase the risk of gastrointestinal bleeding.
  • Corticosteroids: Increased risk of GI ulceration or bleeding.

Missed dose

  • If you miss a dose of Voveran, take it as soon as you remember.
  • However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule.
  • Do not take a double dose to make up for a missed one.

Overdose

Symptoms of overdose may include nausea, vomiting, epigastric pain, gastrointestinal bleeding, drowsiness, dizziness, tinnitus, syncope, or rarely, hypertension, acute renal failure, respiratory depression, and coma. There is no specific antidote. Management is supportive and includes gastric lavage or activated charcoal (if presented early) and close monitoring of vital organ function. Forced diuresis, hemodialysis, or hemoperfusion are unlikely to be beneficial due to the high protein binding of diclofenac.

Storage

  • Store at room temperature (15°C to 30°C or 59°F to 86°F) away from light, moisture, and heat.
  • Do not freeze.
  • Keep all medicines out of the reach of children and pets.
  • Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Dispose of unused medication through a medicine take-back program.
