It is also contraindicated in patients who are using guanylate cyclase stimulators, such as riociguat. Avoid use in patients with severe hepatic impairment, severe hypotension (BP <90/50 mmHg), recent stroke or myocardial infarction, unstable angina, or life-threatening arrhythmias.

Possible side effect

Common adverse reactions (≥2%) include headache, flushing, nasal congestion, dyspepsia, and dizziness. Less frequently, visual disturbances (e.g., blurred vision, changes in color perception), back pain, myalgia, and nausea may occur. Priapism (a prolonged and painful erection lasting more than 4 hours) is a rare but serious adverse event that requires immediate medical attention to avoid permanent damage to penile tissues. Sudden decrease or loss of hearing, or tinnitus, has been reported in association with PDE5 inhibitor use. Allergic reactions, including rash and urticaria, may occur.

Drug interaction

Concomitant use with nitrates or nitric oxide donors is contraindicated. Alpha-blockers (e.g., doxazosin, tamsulosin) may potentiate the blood-pressure-lowering effects; co-administration should be approached with caution. CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, erythromycin) can increase udenafil plasma concentrations; dose adjustment may be necessary. Conversely, CYP3A4 inducers (e.g., rifampin, carbamazepine) may decrease its efficacy. Use with other antihypertensive agents may add to the hypotensive effect. Alcohol may increase the risk of orthostatic hypotension and dizziness.

Missed dose

Zudena is taken on an as-needed basis, not on a fixed schedule. Therefore, the concept of a “missed dose” does not apply. Do not take an extra tablet to make up for a missed opportunity. Adhere to the dosing instruction of not exceeding one tablet in a 24-hour period.

Overdose

In case of suspected overdose, seek immediate medical attention. Overdose may exaggerate known adverse effects such as severe headache, flushing, hypotension, syncope, and potentially prolonged erection. Standard supportive measures should be employed. Hemodialysis is not expected to significantly accelerate clearance, as udenafil is highly protein-bound and extensively metabolized.

Storage

Store at room temperature (15-30°C or 59-86°F). Keep in the original blister package to protect from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
