Common use

Diovan is primarily indicated for the treatment of hypertension in adults and children aged 6 years and older. It is also used in adults for the treatment of heart failure (NYHA Class II–IV), and to reduce cardiovascular mortality in clinically stable patients with left ventricular failure or dysfunction following myocardial infarction. In some cases, it may be used off-label under specialist supervision for other conditions involving the renin-angiotensin-aldosterone system.

Dosage and direction

For hypertension in adults, the usual starting dose is 80 mg or 160 mg once daily. The dosage may be increased to a maximum of 320 mg once daily based on blood pressure response. For heart failure, the recommended starting dose is 40 mg twice daily, which may be titrated to 80 mg or 160 mg twice daily as tolerated. For post-myocardial infarction, therapy may be initiated as early as 12 hours after a heart attack, starting at 20 mg twice daily and titrated to 160 mg twice daily as tolerated. Tablets should be taken with or without food, at approximately the same time each day. Dosage in pediatric hypertensive patients (6–16 years) is weight-based.

Precautions

Patients should be monitored for hypotension, especially after initial dosing or dose increases. Renal function should be assessed prior to and during treatment, particularly in patients with renal impairment, heart failure, or volume depletion. Potassium levels should be monitored in patients with renal impairment, diabetes, or those taking potassium supplements or potassium-sparing diuretics. Use with caution in patients with hepatic impairment. Not recommended during pregnancy due to potential fetal harm. Avoid use in patients with biliary cirrhosis or obstruction.

Contraindications

Diovan is contraindicated in patients with known hypersensitivity to valsartan or any component of the formulation. It is contraindicated in pregnancy, particularly during the second and third trimesters, due to the risk of fetal injury and death. Concomitant use with aliskiren in patients with diabetes is also contraindicated.

Possible side effect

Common side effects may include dizziness, headache, fatigue, abdominal pain, diarrhea, nausea, viral infections, upper respiratory tract infection, cough, and joint pain. Less frequently, orthostatic hypotension, hyperkalemia, impaired renal function, angioedema, rash, and decreased hemoglobin may occur. Rare but serious side effects include severe hypotension, acute renal failure, and hepatotoxicity.

Drug interaction

Diovan may interact with potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium, increasing the risk of hyperkalemia. Concurrent use with NSAIDs may reduce the antihypertensive effect and increase the risk of renal impairment. Lithium levels may increase when co-administered with Diovan. Use with other antihypertensive agents may potentiate hypotensive effects. Avoid concomitant use with aliskiren in diabetic patients.

Missed dose

If a dose is missed, it should be taken as soon as remembered unless it is almost time for the next scheduled dose. In that case, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one.

Overdose

Symptoms of overdose may include severe hypotension and tachycardia. Bradycardia may also occur. In case of suspected overdose, symptomatic and supportive treatment should be initiated, including intravenous fluids and vasoconstrictors if necessary. Valsartan is not significantly removed by hemodialysis.

Storage

Store at room temperature (20–25Β°C or 68–77Β°F), in a dry place, protected from light and moisture. Keep in the original container with the lid tightly closed. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.
